Multidetector computed tomography (MDCT) is a sensitive and specific tool and should become standard treatment for the diagnosis of suspected appendicitis in adults, according to an analysis of the use of MDCT studies reported in the Annals of Internal Medicine in June 2011.
In the study, conducted assessments manfaatdiagnostik MDCT for the diagnosis of suspected acute appendicitis in adults. From studies conducted retrospektf U.S. academic medical centers, found as many as 2871 consecutive adults referred for MDCT performed with suspicion of appendicitis from January 2000 until December 2009. Radiologist who was informed of indications interpreted not focus with
abdominopelvic MDCT. Using recent data from clinical findings, surgical, and pathology combined as the reference standard, the researchers determined the diagnostic performance of MDCT for acute appendicitis.
Acute appendicitis was confirmed in 675 (23.5%) of 2871 patients. For the diagnosis of acute appendicitis by MDCT, the sensitivity of 98.5% (665/675 patients), and specificity was 98.0% (2153/2196 patients). Negative and positive predictive value of MDCT, respectively 99.5% (2153/2163 patients) and 93.9% (665/708 patients). Positive likelihood ratio of 51.3 (95% CI, 38.1 to 69.0), and negative likelihood ratio was 0.015 (95% CI, 0.008 to 0.028).
Limitations of this study include the retrospective nature of studies, there is no comparison as in cohort studies, referral bias, but in general the use of routine use of MDCT is quite helpful as a diagnostic standard of suspicion buntuk bowel prior to surgery.
